Do not place resident-identifying information, diagnoses, medication lists, records, images, or clinical documents into a public inquiry form. The public forms are limited to general scope and contact information.
Authorization and access
Record review and facility access require valid resident or representative authorization and remain subject to applicable privacy, resident-rights, safety, and facility procedures.
Secure transfer is arranged separately
When authorized records are required for a defined engagement, Care Reliability Partners provides separate secure-transfer instructions. The absence of a public upload field is intentional.
Independent family engagement
Family services are independently retained by the resident or family and do not make Care Reliability Partners an employee, agent, surveyor, or representative of the facility.
Separate facility work
Resident-specific information obtained during a family engagement is not used as a sales asset for a facility consulting engagement. Any later facility engagement requires a separate agreement, separate scope, and separate authorization.
